Comments:
An unannounced monitoring inspection was conducted beginning at 9:15 a.m. and ending at 11:15 a.m. with the provider and assistant. There were nine children in care ranging in age from seventeen months to four years old, twenty points. There were three resident children present ages seven, six and two and the assistant's two children, ages eight for a total of twenty-five points. Resident children and the assistant's children were present due to the weather-related school delay and went upstairs at the beginning of the inspection. Children were observed participating in lesson. Younger children had free play with their choice of toys. After lessons, children created an art project based upon the lesson of the day. All children then had music time that included a dance to 'Baby Shark', other songs, instruments and sensory toys. Four children's records, three caregiver records and one household member record were reviewed. The provider is no longer Medication Administration Training (MAT) certified and cannot administer medication to children in care. Description: Based on review of four children's records, the provider failed to obtain documentation of physical within 30 days of attendance. Evidence: Child #3, first day of attendance 10/2/2018, did not have documentation of a physical in the record.

Plan of Correction: The information will be requested from the parents again

Standard #: 22VAC40-111-60-B
Description: Based on review of four children's records, not all records were complete. Evidence: Child #1, first day of attendance 9/1/16, and Child #2, first day of attendance 10/1/2018, did not have a full address for a second emergency contact. Child #3, first day of attendance 10/2/2018, did not have a second emergency contact.

Plan of Correction: The information will be requested from the parents again.
