22VAC40-191 Background Checks for Child Welfare Agencies
An unannounced monitoring inspection was completed on January 25, 2019, from approximately 1:00p.m. to 3:45p.m. Three children were in attendance, ages twenty-three months to three years old, for a total of seven points. The children were observed interacting with the provider during toileting, hand washing, play time and nap time. The physical environment and equipment and the first aid kit and emergency supplies were inspected. Five children?s records, two caregiver records, one household member's record, the emergency preparedness and response plan, fire drill, shelter in place and lock down drill logs, daily attendance and required postings were reviewed. There were no injury reports to review. The provider was available for interview, the inspection and the exit interview at which time inspection findings were reviewed. There were three citations for violations of the Standards for Family Day Homes. See the violation notice on the Department's public web site for violations of the Standards.
Standard #: 22VAC40-111-100-A Description: Based on a review of children?s records, the licensee failed to obtain documentation of a physical examination prior to the child?s attendance or within 30 days after the first day of attendance for one of five children.
Evidence: 1. Child #1 was enrolled on November 7, 2018. 2. Child #1?s record lacked a physical. 3. Caregiver #1 acknowledged the physical was not obtained prior to attendance or within 30 days after the first day of attendance.
Plan of Correction: The provider will obtain documentation of the physical from the parent.
Standard #: 22VAC40-111-70-A Description: Based on a review of children?s records, the licensee failed to ensure that before the first day of attendance, parents were provided all the required information in writing for one of five children. Evidence: 1. Child #1 was enrolled on November 7, 2018. 2. Child #1's record lacked documentation of written notification regarding whether or not the provider has liability insurance in force on the family day home. 2. Caregiver #1 acknowledged this information was not provided to Child #1's parent yet. Plan of Correction: The provider will provide written notice to the parent.
Standard #: 22VAC40-111-180-A Description: Based on a review of caregiver and household member records, the licensee failed to ensure that each caregiver and adult household member obtain a tuberculosis screening every two years for one of two caregivers and one of one adult household member. Evidence: 1. Caregiver #2's record contained tuberculosis screenings dated May 17, 2016 and July 24, 2018. 2. HHM #1's record contained a tuberculosis screening dated May 9, 2016. 2. Caregiver #1 acknowledged these screening were not obtained within the required time frames. Plan of Correction: The provider will require household member to obtain an updated tuberculosis screening. The caregiver's tuberculosis's screenings cannot be corrected. The provider will continue to review records at the beginning of each year to ensure screenings are obtained timely.
