Comments:
An unannounced renewal Inspection was conducted on Thursday, January 24, 2019 from 10:00am to 12:45pm to determine the provider?s compliance with licensing standards. During today?s inspection, there were a total of 11 children in care totaling 34 points with three caregivers. Upon the inspector's arrival, the children were observed during circle time, singing songs, and interacting with peers and caregivers. The caregivers were observed assisting the children during bathroom breaks and washing hands. Later, the children were observed during lunch. The provider was observed having positive interactions with the children throughout the inspection. All areas used for child care were inspected. The family day home has age and stage appropriate materials and supplies. The required information was posted to include: the license, inspection notice, emergency numbers, daily schedule, menu and various parent information. The first aid kit and non-emergency supplies were inspected and found complete. Three caregiver records, one household member's records and five children's records were reviewed. There were no children with food allergies, food sensitivities, or dietary restrictions. Description: Based on a review of five children's records on 01/24/2019, the provider did not obtain documentation of two children's physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: 1) The record for Child #3 (DOE: 11/26/2018) did not contain a physical examination. 2) The record for Child #3 (DOE: 10/03/2018) did not contain a physical examination.

Plan of Correction: Per the Provider: Will put physical in report today. Going forward, the provider will ensure the documentation is in the children's records as required.

Standard #: 22VAC40-111-60-B
Description: Based on a review of five children's records on 01/24/2019, the provider did not ensure five out of five records contained the required information. Evidence: 1) The record for Child #1, Child #2, Child #3, Child #4, and Child #5 did not contain the parent's signed acknowledgement of the receipt of the information required by 22 VAC 40-111-70. Child #1 (DOE: 01/07/2019) was missing a signed information for parents form, the family day home's policy for the administration of medications, and the family day home's liability insurance declaration; Child #2 (DOE: 11/26/2018) was missing a signed information for parents form and the family day home's liability insurance declaration; Child #3 (DOE: 10/03/2018) was missing a signed information for parents form, the family day home's policy for the administration of medications, and the family day home's liability insurance declaration; Child #4 (DOE: 01/02/2019) was missing the family day home's liability insurance declaration; and Child #5 (DOE: 11/02/2018) was missing the family day home's policy for the administration of medications, and the family day home's liability declaration. 2) The record for Child #4 (DOE: 01/02/2019) did not contain the address and phone number for a second emergency contact. 3) The record for Child #2 (DOE: 11/26/2018) did not contain the policy number of the child's medical insurance. 4) The record for Child #5 (DOE: 11/12/2018) did not contain the policy number of the child's medical insurance.

Plan of Correction: Per the Provider: Will put information in the file. Going forward, the provider will ensure the information is signed prior to enrolling a child.

Standard #: 22VAC40-111-90-A
Description: Based on a review of five children's records on 01/24/2019, the provider did not obtain documentation that two children had been adequately immunized before the children may attend the family day home. Evidence: 1) The record for Child #2 (DOE: 11/26/2018) contained immunization records dated 11/30/2018. 2) The record for Child #3 (DOE: 10/03/2018) did not contain an immunization record.

Plan of Correction: Per the Provider: Will put immunization in report today. Going forward, the provider will ensure the immunization records are obtained prior to enrolling a child.

Standard #: 63.2-1721.1-B-2
Description: Based on a review of documentation on 01/24/2019, one adult household member did not have the results of a fingerprint based background check completed by September 30, 2018. Evidence: 1) The record for Household Member #1 contained the results of a fingerprint based background check dated 10/05/2018, exceeding the 09/30/2018 deadline.

Plan of Correction: Already completed.
