Comments:
An unannounced renewal inspection was completed on 1/28/19. There were no children in care. The provider has two children enrolled; care was reviewed at the last inspection. Two children's records, the provider's record, and a substitute provider's record were all reviewed. No medications for children in care. The inspector and the Licensing Administrator arrived at the provider's home at 8:40 am and departed at 10:00 am.

Standard #: 22VAC40-111-60-B
Description: Based on record review, the provider failed to ensure that each child's record contained all required information. Evidence: 1. The record for child #1 was missing the policy number for the child's medical insurance. 2. The provider verified that this information was missing.

Plan of Correction: The provider will have the grandmother document the policy number.
