An unannounced monitoring inspection was conducted today between 10:30am and 11:30am. 10 children (28pts) were present with 2 caregivers. The physical space, caregiver records, 3 children's records, evacuation drills, cribs/mats, the diaper changing area, staff interactions and required posted documents were reviewed. Standard #: 22VAC40-111-830-A
Description: Based on review of records, evacuation drills were not conducted monthly. Evidence: No drills were conducted in November or December of 2018.

Plan of Correction: We will do one today and maintain monthly from here on.

Standard #: 22VAC40-111-250-E
Description: Based on observations, latex balloons were available to children under the age of 5. Evidence: Children (ages infant to 2 years old) were observed dancing with balloons during circle time.

Plan of Correction: Balloons were removed during the inspection. Provider stated they wouldn't use them again.
