54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitoring inspection was conducted today from 11:00am-1:30pm. There were 5 children (16 points) directly supervised by 2 staff. The home, 2 staff records, 5 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, eating lunch, being bottle fed, and napping. Diapering procedures were also observed. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified on the Violation Notice. Standard #: 22VAC40-111-760-A-1 Description: Based on observation, the first aid kit was incomplete. Evidence: The first aid kit is missing one of two required triangular bandages. Plan of Correction: I will make one.
Standard #: 22VAC40-111-830-C Description: Based on review of emergency drills, documentation of evacuation drills was incomplete. Evidence: Documentation of required monthly evacuation drills were not on file for October 2018, November 2018, December 2018 and January 2019. Plan of Correction: I will print a new form.
Standard #: 22VAC40-111-70-A Description: Based on review of children's records, the provider does not have documentation that parents were provided in writing required information before the child's first day of attendance. Evidence: The records of child#1, #2 and #5 are missing the family day home's Liability Insurance Declaration form. Child#4's record is missing the family day home's provisions of the emergency preparedness and prepare plan. Plan of Correction: Today I will print the forms and hand it to them.
