An unannounced renewal study was conducted from 10:30 a.m. to 11:45 a.m. There were 8 children in care with the provider and one assistant ages, 7 months, 12 months, 2 years, 2 years, 2 years, 3 years, 3 years, and 3 years for a total of 20 points. The children were observed participating in free play and preparing for and eating lunch. Lunch served today is chicken nuggets, french fries, bananas, and milk. Eight children's records were reviewed. The provider, assistant and household member records were reviewed and in compliance. There are reportedly no medications being administered to children in the family day home at this time. The provider is current in CPR and First Aid Training. Standard #: 22VAC40-111-870-G Description: Based on observation and interview with the provider it was determined that tables were not cleaned after each use. Evidence: Upon arrival at the family day home at 10:30 a.m. the LI (Licensing Inspector) observed various foods such as bananas which had turned brown, sippy cups, and food debris on the feeding table. The table was not cleaned before the children sat down to eat lunch. Plan of Correction: The table will be cleaned after each use.
Standard #: 22VAC40-111-920 Description: Based on observation of the family day home the menu for the current week was not posted or available for review. Evidence: The most recent menu posted was for the month of January. Plan of Correction: The current menu will be posted.
Standard #: 22VAC40-111-960-E Description: Based on observation and interview it was determined that not all bottles for infants in care are labeled with the child's full name and date. Evidence: The bottle for Child B was not labeled with the child's full name and date. Plan of Correction: All bottles will be labeled with the child's full name and date.
Standard #: 22VAC40-111-100-A Description: Based on review of 8 children's records it was determined that not all records contained documentation of a physical within the first 30 days of enrollment. Evidence: Child A with a start date of 11/30/2018 did not have documentation of a physical on file. Plan of Correction: The provider will request the most recent physical from the parents of Child A.
