An unannounced monitoring inspection was conducted on 02/14/2019 from 10:25am - 11:45am. At the time of entrance there were four children in care with two caregivers. The ages of the children counted for a total of ten points. The infant in care was observed napping in his crib. Two preschool children were observed coloring pictures with the provider while the other preschool child in care was napping on a cot. Lunch was served at 11:40am which consisted of chicken nuggets, peas, yellow rice, fresh pineapple and milk. Records were reviewed for five children, two caregivers and two household members. The violations are listed on the violation notice and were reviewed with the provider at the conclusion of the inspection.
Standard #: 22VAC40-111-90-C Description: Based on record review and interview, the provider failed to obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence: 1. The most recent immunizations for child 1 were administered on 12/22/2016 according to the immunization record on file. a. Child 1 is now over the age of two years and was in care during the inspection. 2. The provider confirmed that the updated immunization record was not on file. Plan of Correction: The parent was contacted during the inspection and asked to bring a copy of the updated immunization record.
Standard #: 22VAC40-111-240-A Description: Based on observation, the provider failed to ensure that outside equipment of the family day home shall be maintained in a safe condition. Evidence: There were two areas of sharp & peeling rust on the bottom of the outdoor swing metal support bars. Plan of Correction: The swing was repaired during the inspection.
