Technical assistance was provided in the following areas of the standards: 22VAC40-191 (Background checks); 22VAC40-111-60-B (Children's records); 22VAC40-185-(2)-120 (Caregiver records)
An unannounced monitoring inspection was conducted on 2/22/19 from 9:45am - 11:15am. At the time of the inspection there were10 children in care, ages two years old to four years old (20 points), with two caregivers present. Records were reviewed for five children and five caregivers. Children were observed during learning activities. Medications, emergency plan and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice and were discussed with the provider during the exit interview.
Standard #: 22VAC40-111-120-B Description: Based on a review of five caregiver's records, it was determined that the licensee did not ensure that a record is maintained for each caregiver that contains all of the required items. Evidence: 1, The record for caregiver #5 did not the include the name, address and telephone number of a person to contact in an emergency. 2. Caregiver #1 reviewed the record for caregiver #5 and confirmed that it did not contain all of the required information. Plan of Correction: The provider responded: Caregiver #5 will be asked to provide an emergency contact. The records for all newly hire caregivers will be checked prior to their first day of employment in the family day home.
Standard #: 22VAC40-111-60-B Description: Based on a review of five children's records, it was determined that the licensee did not ensure that a record is maintained for each child that contains all of the required items. Evidence: 1. The record for child #2 did not contain the date of enrollment and the parent acknowledgement for reviewing the family day home's emergency preparedness plan. 2. Caregiver #1 reviewed the record for child #2 and confirmed that it did not contain all of the required information. Plan of Correction: The provider responded: The parents of child #2 will be asked to provide all missing information. The records for all newly enrolled children will be checked prior to their first day of care in the family day home.
