Reminder: 16 hours of annual training is required for both the Provider and Assistant.
An unannounced monitoring inspection was conducted today from approximately 10:15am to 11:50am. 2 children (4pts) were present with 2 caregivers. Ratios were in compliance. The physical space, 4 children's records, 2 caregiver records, emergency evacuation drills, attendance records and required posted documents were reviewed. No medications were on site. Children were observed working on crafts and interacting with the caregivers. Interactions were positive. The space was clean, well organized and contained age appropriate materials. Standard #: 22VAC40-111-100-A Description: Based on interview with the Provider, and review of records, documentation of a physical examination was not available for review. Evidence: The documentation of a physical exam for Child #1 was unavailable for review during today's inspection. Plan of Correction: I will ask the parents to resubmit the documentation if I am unable to find it.
Standard #: 22VAC40-111-60-A Description: Based on record review, 3 out of 4 records reviewed did not contain all required information. In addition, the file for a child present at the FDH could not be found. Evidence: 3 records did not have documentation that an annual review had been completed. Child #1-last checked 4/5/2017, Child#2-last checked 7/6/2017, Child #3-last checked 4/17/2017. The file for Child #4 could not be found today. The file was reviewed at a previous inspection but the Provider could not locate it during today's inspection. Plan of Correction: I will have the parents review the emergency information and update the documentation right away. I will also find the missing file or ask the parents to provide me with a new one, if needed.
Standard #: 22VAC40-111-70-A Description: Based on Provider interview and review of records, documentation that information for parents, provided in writing, was not available for review. Evidence: The file for Child #4 was unavailable for review during today's inspection. Plan of Correction: I will obtain the new information right away.
Standard #: 22VAC40-111-180-A Description: Based on review of records, the Provider did not obtain subsequent tuberculosis screenings for each caregiver. Evidence: The Provider's TB test expired 09/28/2018 and the Assistant's TB test expired 01/10/2019. Plan of Correction: We will both get the TB tests done right away.
