Discussion about new renewal application.
An unannounced monitoring inspection was conducted today from 10:25am-1:15pm. There were 6 children (18 points) directly supervised by the provider. The home, 1 staff record, 6 children?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing freely, learning about the planets, doing worksheets, coloring, and eating snack. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified on the Violation Notice. Standard #: 22VAC40-111-830-C Description: Based on record review, shelter-in-place drills were not practiced twice a year. Evidence: There was documentation of one shelter in place practiced in April 2018, a second shelter in place drill was not documented in 2018. Plan of Correction: We will practice an additional shelter-in-place drill tomorrow.
Standard #: 22VAC40-111-960-E Description: Based on observation, it was determined that infant's bottles were not labeled with the child's full name and date. Evidence: 5 infant bottles were observed in the refrigerator not labeled with the child's name and date. Plan of Correction: I will talk to the parents to label their bottles, if not I will label them myself.
Standard #: 22VAC40-111-100-A Description: Based on record review, it was determined that the provider did not have documentation of a physical for each enrolled child. Evidence: The records of Child#2, Child#4 and Child#6 did not have a physical examination on file within 30 days of attendance. Child#2's first day of attendance was documented as 3/6/2018, Child#4's first day of attendance was documented as 11/17/2014 and Child#6's first day of attendance was documented as 8/13/2018. Plan of Correction: I will get the missing information from the parents.
Standard #: 22VAC40-111-60-A Description: Based on record review, it was determined that children's records were incomplete. Evidence: Child#2's record is missing the mother's employment information. Child#4's record is missing the father's work telephone number and his work address. Plan of Correction: I will get the missing information from the parents.
Standard #: 22VAC40-111-80-A Description: Based on record review, the family day home did not have documentation that they had view the proof of identity for a child in care within seven days of the child's first day of attendance at the family day home. Evidence: Child#1's record does not have documentation that the provider verified the child's proof of identity within seven business days of the child's first day of attendance. The chid's first day of attendance was documented as 2/4/2019. Plan of Correction: I will get the missing information from the parents.
Standard #: 22VAC40-111-90-A Description: Based on record review, it was determined that the provider did not obtain documentation that each child has been adequately immunized before the child's first day of attendance. Evidence: The records of Child#2 and Child#4 do not contain immunization documentation. Plan of Correction: I will get the missing information from the parents.
Standard #: 22VAC40-111-570-A Description: Based on observation, the provider did not ensure that they did not exceed 16 points per caregiver. Evidence: During the inspection the provider was caring for 6 children, ages 1 - 6 month old (4 points), 1 - 8 month old (4 points), 1 - 15 month old (4 points), 1 - 3 year old (2 points), and 2 - 4 years old (4 points). Based on the children's date of birth this adds up to 18 points, the provider needed an additional caregiver. Plan of Correction: I have someone coming on Wednesday and Friday and will find coverage for the rest of the week.
