Comments:
An announed initial inspection was completed on 2/27/19. The application for licensure was received 12/28/18. The provider's record and an assistant's record were reviewed during the inspection. Four household member's records were also reviewed. All areas of the home were viewed. There were two children in care with the applicant (provider) and an assistant present. The provider does not administer medications. The inspection protocol brochure and risk assessment matrix was provided and explained to the provider. The inspector arrived at the provider's home at 1:00 pm and departed at 4:15 pm. There was discussion with the provider about the following: the menu, infant care including crib/sleeping requirements, all requirements for an assistant's record, detailed information about emergency preparedness, annual requirements, conditional license process.

Violations:
Standard #: 22VAC40-111-790
Description: Based on observation and interview, the provider failed to ensure that the phone number for the responsible person for emergency backup care as required in 22 VAC 40-111-800 A 3 was posted. Evidence: 1. The back-up caregiver's number was not observed. 2. The provider was asked about the phone number location and the provider stated that it was not posted.

Plan of Correction: The provider added the number of the back-up caregiver to the emergency contact list during the inspection.

Standard #: 22VAC40-111-800-A
Description: Based on review of policies and interview with the provider, the provider failed to ensure that there was a complete emergency preparedness plan with all components required by the Standards. Evidence: 1. The provider's emergency plan was observed. There were several components that were not observed that are required by the Standards (emergency back-up caregiver, method of ensuring all are evacuated/arrive to emergency location, routes to the relocation sites). 2. The provider stated that these components were not in the plan.

Plan of Correction: The provider printed the Department's model form and will complete it to ensure all components are addressed.

Standard #: 22VAC40-111-320-G
Description: Based on interview with the provider, the provider failed to ensure that wood burning stoves and fireplaces and associated chimneys was inspected annually by a knowledgeable inspector to verify that the devices are properly installed, maintained, and cleaned as needed. Documentation of the inspection and cleaning shall be maintained by the provider. Evidence: 1. A wood burning stove was observed. The provider was asked if this was a wood burning stove in which the provider verified that it was. The provider was asked if the stove and chimney had been inspected. The provider stated that it had not been inspected since living in the home.

Plan of Correction: The provider called a chimney sweep service during the inspection to make an appointment to have an inspection completed. Documentation will be kept of this inspection and done annually.
