Technical Assistance:
The VA State Police Criminal Record Check does not need to be completed as the Fingerprint process covers the same information. Virginia will continue to cover the cost of fingerprinting until 9/30/2019. Older children should be seated at a table rather than in highchairs to prevent the chair tipping over if the child weighs too much. Please check the manufacturer's guidelines for weight restrictions.

Comments:
An unannounced monitoring visit was conducted today from 10:55-12:15. Nine children were present with three caregivers for twenty six points which is within ratios for supervision. Observed children transitioning from dancing, snack, reading and free play. Meals and snacks are bought from home. Records for 2 children as well as a new assistant were reviewed as well as emergency drills, supplies, written plan and trainings (First Aid/CPR/Medication Administration Training. No medication was reported to be on-site. Over the counter products and food from home were labeled to identify the owner. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 22VAC40-111-100-A
Description: Based on record review, a child did not have documentation of a physical exam within 30 days of attending the day care. Evidence: Child #1's first day of attendance was 9/5/2018 and is missing the physical form completed by the physician.

Plan of Correction: Parent will return the form within the week.

Standard #: 22VAC40-111-90-C
Description: Based on record review, 1 of 2 children's records reviewed did not have an updated immunization every six months for children under two years of age. Evidence: Child #1's last immunization record was dated 8/13/2018.

Plan of Correction: The immunization record was returned to the parent as it was not stamped. Parent will bring back the form within the week.
