An unannounced monitoring inspection was completed on March 1, 2019 from approximately 10:15a.m. to 11:50a.m. Ten children were in care, ages infant to four years old, for a total of twenty-five points. The children were observed interacting with the provider and three assistants, during center time, activities for infants, singing and dancing, hand washing, toileting, and diapering. Children?s learning materials and equipment, the bathroom, inside areas and nutrition were inspected. The provider administers medications but there are currently no children taking medications. Five children?s records, four caregiver records, one household member's record, fire drill and shelter in place logs, medication administration certifications, the emergency preparedness and response plan, daily attendance and required postings were reviewed. There were no injury reports to review. The provider was available for interview, inspection and exit interview at which time inspection findings were reviewed. There was one citation for a violation of the Standards for Licensed Family Day Homes. See the violation notice on the Department's public web site for violations of the Standards.

Standard #: 22VAC40-111-170-A
Description: Based on review of staff records and interview, the licensee failed to obtain from each caregiver at the time of hire and prior to coming into contact with children a current tuberculosis screening. Evidence: 1. Caregiver #2's record, hired on October 8, 2018, contained a tuberculosis screening dated November 3, 2018 and therefore not within the requited time frame. 2. The provider indicated a misunderstanding of the Standard and expressed she now understands the time frame requirement.

Plan of Correction: The provider will ensure that tuberculosis screenings are obtained at hire in the future.
