An unannounced monitoring inspection was conducted today. The provider and one assistant caregiver were caring for 7 enrolled children and one resident child, totaling 20 points. Children's records, children's medications, emergency preparedness items, the physical plant, programming, and children's nutrition were inspected today. This inspection took place in the middle of the day. The children were observed playing with playdoh, listening to stories, and taking their afternoon nap. The children had been served hummus, carrots, grilled cheese, bell peppers, and milk for lunch earlier today. This inspection was completed between approximately 12:15pm and 1:30pm. Standard #: 22VAC40-111-700-A Description: Based on a review of children's medications, it was determined that one medication has expired. Evidence: One child's prescription medication expired on 1/11/19. Plan of Correction: I have asked the parents many times to bring new medications. I will speak with them again today and give a very clear one week deadline.
Standard #: 22VAC40-111-700-C Description: Based on a review of children's medications, it was determined that one child's medications were not sent home within 14 days after the expiration of the authorization form. Evidence: The authorization for one child's medications expired on 7/3/18. The provider neither returned these medications nor obtained a renewed authorization form within 14 days past expiration. Plan of Correction: The parent has been asked many times to bring me a new authorization form. I will speak to the parent again today and give them a very clear one week deadline.
Standard #: 22VAC40-111-900-A Description: Based on interviews, it was determined that the provider served one food item in a way that did not adequately assist in preventing choking. Evidence: It was reported to the Inspector that children under the age of 4 were served baby carrots for lunch. The carrots were neither cut nor cooked. Plan of Correction: When we serve carrots we will either slice them or cook them.
