- Technical Assistance:
Use of the foot operated diaper disposal system was discussed with the provider. It was reviewed with the provider that any substitutions to the menu items must be indicated on the posted menu. All foods served to the children while in care must be indicted on the menu. The menu is to be kept for a one week period.
An unannounced, mandated renewal inspection was conducted today. The inspector arrived at 9:05 AM and departed at 12:20 PM. Two children,totaling 4 points, were in care with one caregiver. Two caregiver/household records were reviewed. Four children's records were reviewed. It was reported that no prescription medication has been administered to children in care. No children's injuries were reported. An exit meeting was conducted with the provider prior to closure of the inspection.
Standard #: 22VAC40-111-100-A Description: Based upon review of four children's records and provider interview, the provider has not obtained documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: 1. Child 2, 3 and 4 began attending the first day of licensure, September 4, 2018. Results of physical examinations were not available in any of the three children's records. 2. The provider verified that physical examination records were not available for child 2, 3 or 4. Plan of Correction: The provider responded with the following: Physical examinations will be obtained for all children before attending or within 30 days following enrollment.
Standard #: 22VAC40-111-50-A Description: Based upon observation, the provider has not ensured that there is a written record of children in attendance each day. Evidence: 1. There was no attendance record for today. There were two children in care. 2. The provider stated that today was to be a play date. Plan of Correction: The provider responded with the following: As a licensed provider, I now understand that when enrolled children are in care, it is not a play date, but a part of my licensed program.
Standard #: 22VAC40-111-60-A Description: Based upon review of attendance records and provider interview, the provider has not ensured that there is a record at the family day home for each enrolled child. Evidence: 1. According to the attendance records provided, child 5 attended February 25, 28 and March 1st. There were no records available for child 5. 2. The provider stated that the parent did not complete records for child 5. Plan of Correction: The provider responded with the following: No children will be in attendance until complete records have been completed by the parent(s) and submitted to the provider.
Standard #: 22VAC40-111-60-B Description: Based upon review of four children's records, the provider has not ensured that each child's record includes the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached. Evidence: 1. The records for child 2, child 3 and child 4 have indicated only one designated person to be contacted in case of an emergency if a parent cannot be reached. 2. The provider verified that the records for children 2, 3 and 4 have only one designated emergency contact person. Plan of Correction: The provider responded with the following: The parent will be required to provide the name, address and telephone number for at least two designated persons to be contacted when a parent cannot be reached.
Standard #: 22VAC40-111-70-A Description: Based upon review of four children's records, the provider's policies for termination of care were not provided to the parents. Evidence: 1. The information regarding the provider's policy for termination of care was not completed on the parent information pages in any of the four children's records reviewed. 2. The provider acknowledged that the policy for termination of care was not completed on the parent information pages. Plan of Correction: The provider responded with the following: My policies regarding termination of care will be completed and given to all parents.
