An announced initial inspection was conducted on March 12, 2019 from 11:15am to 1:41pm. There were no children in care during this inspection as the home has not yet opened for child care purposes. The family day home is requesting a licensed capacity for 12 children ages 1 day through 12 years. Provider has been approved by zoning to provide care for this number. All required documents including the menu, emergency phone numbers, daily schedule, emergency preparedness plan were posted. First aid kit and emergency equipment were present. The program will offer meals and snacks to children in care. No medication will be administered. Transportation is not provided. The provider has submitted all of the required application materials at this time. During the inspection, compliance with some standards could not be determined since the center is not yet in operation. Compliance with these standards will be determined during unannounced visits once the program is in operation. Though some standards of care could not be determined during the initial study, the center is in substantial compliance with the standards that could be determined. Standard #: 22VAC40-111-730-D
Description: Based on observation, the provider did not ensure that medication was kept in a locked place using a safe locking method that prevents access by children. Evidence: The inspector observed an unlocked cabinet containing several medications. The cabinet was out of the reach of children.

Plan of Correction: Corrected on visit.

Standard #: 22VAC40-111-260-B
Description: Based on observation, the provider did not ensure that a non-climbable barrier at least four feet high was located within 30 feet of drowning hazards. Evidence: The provider's back yard had a fence that measured 3 feet and 10 inches to 4 feet tall in various areas. There was a lake located about 2 1/2 to 3 feet from the provider's fence. The fence had a black material attached to it which made the fence non-climbable.

Plan of Correction: To be corrected with new fence as soon as possible but the children will play only in the front yard until the fence is fixed.

Standard #: 22VAC40-111-350-E
Description: Based on observation, the hot water temperature exceeded the required temperature of 120?F. Evidence: During the inspection, the licensing inspector used a digital thermometer to obtain the water temperature at the hand washing sink located in the provider's bathroom. The inspector checked the water temperature three separate times and the provider attempted to turn the hot water temperature down during the inspection. The water temperature at the hand washing sink registered 126.8?F, 126.2? and 124.6?.

Plan of Correction: Was turned down on visit but not low enough.

Standard #: 22VAC40-111-460-C
Description: Based on record review, the provider did not ensure that one of three pets in the home had documentation of a rabies vaccination. Evidence: The provider was not able to provide documentation of up to date vaccinations for the home's cat.
