An unannounced monitoring inspection took place between the hours of 12:05pm-2:00pm. There was a total of 6 children (13 points) in the direct care of 1 Provider within the supervision guidelines. A total of 5 children?s records and 2 Provider/household member?s files were reviewed. The first aid kit, emergency procedures, evacuation drills, and injury reports were also reviewed. There were no medications on-site during the inspection. The children were engaged in the following: playing outside, playing with toys, toileting and other organized activities. Standard #: 22VAC40-111-100-A
Description: Based on record review, 1 of 5 children's records was missing documentation of a physical examination prior to a child's attendance of within 30 days after the first day of attendance. Evidence: 1. Child #5 (start date: 10/3/2016) did not have documentation of a physical examination on file.

Plan of Correction: I will ask the parents to bring it in when they pick-up today.

Description: Based on record review, 3 of 5 children's records were not up-to-date. Evidence: 1. Child #2 was missing an annual update for their emergency information. The last update was made on 9/19/2017. 2. Child #4 was missing an annual update for their emergency information. The last update was made on 9/19/2017. 3. Child #5 was missing an annual update for their emergency information. The last update was made on 9/19/2017.

Plan of Correction: I will ask all the parents to update their information today at pick-up.
