Thank you for your time and assistance with the unannounced monitoring inspection conducted today, April 5, 2019. The on-site inspection was from 10:25 a.m. to 1:10 p.m. At the time of arrival, there were two children in the care of the provider and one assistant (5 points). There were a total of six children enrolled in the daycare. The exceptions to compliance were noted on the violation notice and the risk ratings for the violations were noted on the supplemental page. There were five children's records reviewed, and the provider and assistant's records were checked for updates. Two providers were interviewed. As is the policy of the provider, there were no medications being administered. Some items observed and reviewed during the inspection included: required posted information, first aid kit, emergency supplies, warm water temperature, fridge and freezer temperatures, daily attendance chart, menu, documentation of evacuation and shelter-in-place drills, morning snack, free play, story time, bathroom and hand washing procedures, diaper changing procedures, and lunch. Standard #: 22VAC40-111-100-A Description: Based on review of five children's records and verification from the provider, one child's record did not contain a physical examination report. Evidence: 1. The record of Child #3 did not contain documentation that a physical examination had been obtained. Date of enrollment for Child #3 was 01/07/2019. Plan of Correction: The provider will talk with the parent of the child and obtain a physical exam as soon as possible.
Standard #: 22VAC40-111-100-B Description: Based on review of five children's records and confirmation from the provider, the physical examination for one child was not conducted within the required time frame. Evidence: 1. The record of Child #1 contained documentation of a physical examination dated 10/05/2018 which was over three months prior to the child's date of enrollment. Child #1 was 13 months old at the time of enrollment and the date of enrollment for Child #1 was 03/15/2019. Plan of Correction: The provider will talk with the parent of the child and obtain a more recent physical exam as soon as possible.
