Comments:
An unannounced monitoring inspection was conducted today from 11:00am-1:45pm. There were 9 children (19 points) directly supervised by 2 staff. The home, 2 staff records, 5 children?s records, 1 household member?s record, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing on the backyard, and eating lunch. Today's lunch consisted of chicken, beans, broccoli, brown rice and milk. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified on the Violation Notice. Violations:
Standard #: 22VAC40-111-40-A
Description: Based on direct observation and record review, the family day home did not ensure that the total number of children receiving care at one time does not exceed the maximum licensed capacity of the home. Evidence: During the inspection there were 9 children in care. The capacity for the family day home is 7 children. The provider is applying for a special exception that would allow for a maximum of 12 children, but the special exception has not yet been approved.

Plan of Correction: In a couple of weeks I will be attending a public hearing that may allow me to care for more than 7 children, possibly 12. I will talk to the families to see if they could wait for the space to become available in a couple of weeks.

Standard #: 22VAC40-111-50-A
Description: Based on record review, the family day home did not keep a written record of children in attendance each day. Evidence: The provider did not have records of daily attendance from 4/10/2019 - 4/22/2019.

Plan of Correction: We will make sure that we keep written attendance every day.

Standard #: 22VAC40-111-90-C
Description: Based on record review, the family day home did not obtain additional immunizations once every six months for children under the age of two years. Evidence: Child#4's record has an immunization record on file that was last updated on 10/28/2015. The child was 12 months of age the last time their immunization record was updated. Child#5's record has an immunization record on file that was last updated on 7/8/2017, the child was 15 months of age the last time their immunization record was updated.

Plan of Correction: We will request the information from the parents.

Standard #: 22VAC40-111-430-A
Description: Based on observation, the provider did not ensure that children under two years of age did not have access to stairs without protective barriers. Evidence: The stair opening located in the programming area that leads to the main floor of the home, was observed without a protective barrier. The stairs leading to the back playground has a protective barrier at the top of the stairs, but not at the bottom of the steps.

Plan of Correction: We will install new gates at both stair openings.
