Comments:
An unannounced mandated inspection was conducted from 12:00pm-1:10pm. There were 9 children (16 points) in the direct supervision of the Provider and Assistant Provider. A sample of 4 children's records, 2 staff records, the first aid kit, evacuation drills, one medication and emergency materials were reviewed. The children were engaged in the following: doing an art activity, learning about plants and how they grow, washing hands, eating lunch, and other organized activities. There was an abundance of age appropriate materials in the day care space. Please contact me if you have any questions at shahana.green@dss.virginia.gov .

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, 1 out 4 children's records did not have the required annual update. Evidence: 1. Child #1's last annual update of information was on 3/26/2018.

Plan of Correction: We will ask the parents to update it this evening.
