An announced initial inspection visit was conducted today between the hours of 9:30am through 11:15am with the provider on site. There was 1 child (2 points) with the provider. A sample of 2 children's files and 4 staff/household member's files were reviewed. The children were engaged in the following: toy play, painting, bathroom breaks, handwashing and coloring. Interactions between the child and provider were positive. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. The physical space, emergency supplies, attendance records and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-111-760-A-1 Description: Based on observation, the first aid kit did not contain all of the required items. Evidence: 1. The first aid kit was missing tweezers and a thermometer. Plan of Correction: I will bring it back tomorrow.
Standard #: 22VAC40-111-880 Description: Based on review of the provider's menu that was posted, it did not follow the most recent, age-appropriate nutritional requirements. Evidence: 1. The May 7, 2019 lunch menu listed rice w/beef, bread, peaches and milk and was missing one serving of vegetable. 2. The May 9, 2019 lunch menu listed chicken sandwhich, bread, pineapple and milk and was missing one serving of vegetable. Plan of Correction: I will make sure I have the appropriate servings listed on the menu.
Standard #: 22VAC40-111-70-B Description: Based on record review, the provider did not obtain the parent's written acknowledgement of the receipt of the provisions of the emergency plan before the child's first day of attendance. Evidence: 1. Child #1 began with the provider on 5/09/2019 and did not have the parent's signature on the provisions of the emergency plan form. 2. Child #2 began with the provider on 4/01/2019 and did not have the parent's signature on the provisions of the emergency plan form Plan of Correction: I will have the parent's sign the forms today.
