54.1-3408 Provider must be MAT certified to administer prescription medication.

An unannounced monitoring inspection was conducted beginning at 11:30 a.m. and ending at 3:00 p.m. with the provider. There were four children in care ages 10 months, 13 months, 2 years, 2 years for a total of 12 points. A fifth child arrived during the inspection, age 2 years, for a total of 15 points. Children were observed during outside time. After outside time, children washed hands and prepared for lunch. Lunch was spaghetti with meat sauce, cantaloupe and milk. After lunch, children prepared for rest time. Four children's records, one caregiver record and one household member record were reviewed. Emergency drills were reviewed. One medication was reviewed. Standard #: 22VAC40-111-700-B
Description: Based on review of medication and documentation, the provider failed to ensure that medication authorizations were renewed after 10 working days. Evidence: A proair inhaler was on site for Child #6 and included written permission from the parent that was not dated. The documentation was accompanied by an Asthma Action Plan dated by the doctor on 7/9/2018.

Plan of Correction: A new authorization will be completed this evening. A long term medication authorization will be obtained as soon as possible.

Standard #: 22VAC40-111-830-A
Description: Based on review of documentation, the provider failed to ensure that emergency evacuation drills were practiced monthly. Evidence: There is no fire drill documented for February 2019.

Plan of Correction: A shelter in place drill was practiced in February. In the future, fire drills will be practiced monthly.

Standard #: 22VAC40-111-920
Description: Based on review of the weekly menu, it did not meet requirements. Evidence: The weekly menu was not dated. It did not list the meal served for lunch today. The weekly menu listed chicken wings, apples, kale, brown rice and milk but spaghetti with meat sauce, cantaloupe and milk were served.

Plan of Correction: The weekly menu will dated and updated as needed.

Standard #: 22VAC40-111-340-C
Description: Based on observation, the bathroom was not equipped with paper towels. Evidence: No paper towels were observed in the bathroom. Children were observed leaving the bathroom with wet hands and sitting at the table.

Plan of Correction: Children were given paper towels at the table once they sat down.

Standard #: 22VAC40-111-550
Description: Based on observation, a play pen was not used to according to requirements and not be occupied by more than one child. Evidence: A 12 month old and a 10 month old were observed in a playpen together.

Plan of Correction: Only one child will be placed in the play pen at any one time.
