Comments:
An unannounced mandated Monitoring inspection was conducted today from 11:40am to 12:45pm. There were 3 children present (8 points) with 1 caregiver. Children were observed in bouncy seats, in exer-saucers, playing on the floor with legos, shape manipulatives and rattles. Reviewed 3 children's files, 1 caregiver file, 2 resident files, emergency evacuation drills and supplies, menu, attendance, crib compliance, and physical plant. Standard #: 22VAC40-111-100-A
Description: Based on record review, provider did not obtain documentation of a physical examination by or under the direction of a physician within 30 days after the first day of attendance. Evidence: Child#1 (start date (3/29/19) did not have documentation of a physical within 30 days of first day of care.

Standard #: 22VAC40-111-60-A
Description: Based on record review, children's files were incomplete. Evidence: Child#1 was missing the policy number of the child's medical insurance. Enrollment date 3/26/19.
