63.2 Licensure and Registration Procedures
An unannounced renewal inspection was completed on 5/22/19. There were two children in care with the provider at arrival of the inspector for a total of seven points. Two additional children were picked up from public school on two separate outings. The point total was still seven points as the two school-age children were over the age to count as points. Six children's records were reviewed, the provider's record was reviewed, and one household member's record was reviewed. The provider does not administer medications. The inspector arrived at the provider's home at 2:05 pm and departed at 4:40 pm. The following was reviewed with the provider: a May fire drill will need to be practiced and documented, a lock down drill will need to be practiced for 2019, a second shelter-in-place drill for 2019 will need to be practiced and documented, the emergency preparedness and response plan will need to be reviewed in June 2019.
Standard #: 22VAC40-111-960-B Description: Based on observation, the provider failed to ensure that a bottle for an infant who could not hold his own bottle was not propped. Evidence: 1. A four month old infant was observed in a car seat with a bottle propped by a towel to feed the infant. The provider was engaged in other activities looking for paperwork. Plan of Correction: This was corrected during the inspection. When the inspector noticed the infant with a propped bottle and informed the provider of the Standard requirement, the provider picked up the infant and fed him.
Standard #: 22VAC40-111-60-B Description: REPEAT VIOLATION Based on record review, the provider failed to ensure that all required information was documented in each child's record. Evidence: 1. The record for child #3 was missing an address for two of two required emergency contacts. 2. The record for child #4 was missing an address for one of two required emergency contacts. Plan of Correction: The provider will have the parents document the addresses.
Standard #: 22VAC40-111-600-B Description: Based on observation, the provider failed to ensure that an infant shall spend no more than 30 minutes of consecutive time during waking hours, with the exception of mealtimes, confined in a crib, play pen, high chair or other confining piece of equipment. The intervening time period between confinements shall be at least one hour. Evidence: 1. A four month old infant was observed awake and in a car seat at arrival of the inspector at 2:05 pm. The infant was taken out at approximately 2:30 pm and put directly into a play pen which is also a confining piece of equipment. The infant was taken out of the play pen and returned to the car seat at approximately 2:50 pm. The infant was taken out of the car seat at 3:25 pm. The infant was awake during all of those times while in the car seat and out of the car seat. The infant was in the car seat for more than 30 minutes on two occasions and the times in between confinements were not at least one hour. Plan of Correction: The provider will make sure the infant is not in an confining piece of equipment more than 30 minutes and out of a confining piece of equipment for at least an hour.
Standard #: 22VAC40-80-120-E-2 Description: Based on observation and interview, the provider failed to ensure that the findings from the most recent inspection were posted. Evidence: 1. The "Violation Notice" from the April 2018 inspection was observed to be posted. The last inspection was completed November 2018. 2. The provider was asked about the "Violation Notice" from the last inspection and could not locate the form. Plan of Correction: The provider will post the "Violation Notice" from today's inspection.
