Technical Assistance:
The Provider requested assistance with the on-line procedure for scheduling an appointment for fingerprint background check. The LI assisted with the process and the Provider scheduled an appointment.

Comments:
An unannounced monitoring inspection was conducted in the Family Day Home on July 31, 2018. The Licensing Inspector observed 4 children in care. Based on the ages of the children, infancy to school-age, the points totaled 9. Three of the four children were napping at the time of the Licensing Inspector's arrival. A sample of 4 children's records was reviewed. Partial information was reviewed for the Provider. The children were observed at nap time, free play, afternoon snack and at dismissal. A sample of records was reviewed. The inspection was conducted from approximately 1:30 to 5:00PM. If you have any questions, please call this LI at (540)309-2310. Thank you.

Violations:
Standard #: 22VAC40-111-90-A
Description: Based on documentation reviewed in a sample of children's records and interview with the Provider, the Provider failed to obtain documentation that each child had been adequately immunized according to the requirements of 32.1-46A of the Code of VA and applicable State Board of Health regulations prior to attendance. Evidence: One of four reviewed records did not contain documentation of an immunization record. The record of child #4 noted the first date of attendance as 07/09/18.

Plan of Correction: At the time of enrollment, the Provider will obtain a current immunization record for each child. Documentation will be on file for each child.

Standard #: 22VAC40-111-90-C
Description: Based on a sample review of children's records, the Provider failed to obtain documentation of additional immunizations for a child who is not exempt from the immunization requirements according to subsection B: Once every 6 months for children under the age of two years. Evidence: The immunization record for child #3 is not current based on the age of the child. Current documentation is not available for immunizations received at 12 and 18 months.

Plan of Correction: The Provider will request an updated immunization record from the parent, documentation will kept in the child's record.

Standard #: 22VAC40-111-280
Description: Based on direct observations, the Provider failed to store potentially poisonous materials in areas inaccessible to children. Evidence: The LI observed multiple personal health and beauty aid items in the bathroom within reach of children.

Plan of Correction: The Provider will keep all personal items in the bathroom out of the reach of children.

Standard #: 22VAC40-111-320-B
Description: Based on direct observations, the Provider failed to ensure child resistant protective covers larger than 1 1/4 inches in diameter were installed on all unused electrical outlets accessible to children under five years of age. Evidence: The LI observed an uncovered electrical wall outlet at the bathroom sink.

Plan of Correction: The Provider will keep all electric outlets covered.

Standard #: 22VAC40-111-460-I
Description: Based on direct observations, the Provider failed to ensure an animal's food and water dishes were inaccessible to children. Evidence: The LI observed the dog's food and water dishes sitting on the floor in the kitchen.

Plan of Correction: The dog's dish will be kept in an area away from the children's area.
