A Subsidy Health and Safety inspection was conducted on 05/17/2019 from 8:55am to 1:15pm to review supplemental health and safety requirements. The children were playing outside upon the inspector?s arrival. The children were later observed playing with toys, watching television and eating lunch. Today, there were seven children, totaling 18 points, present with three caregivers. Interviews were held with the caregivers throughout the inspection. The provider does not administer medication. The provider does not provide transportation. The provider does have a pet. Five children?s records, one caregiver record and two caregiver/household member records were reviewed during the inspection. Last emergency drill: 04/16/2019 Last shelter-in-place drill: 04/26/2019 Last lockdown drill: 05/28/2018 A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. One violation was found during the supplemental subsidy health and safety inspection and is identified on the violation notice. Description: Based on review, the provider did not ensure that all caregivers have orientation training completed on the following specific topics: playground safety, supervision, lost/missing children, ill/injured children, medication administration, natural/man-made disasters, shaken baby, safe sleep, and food. Evidence: The record of caregiver #1 (start date: 7/13/10), caregiver #2 (start date: 01/03/03), and caregiver #3 (start date: 4/1/2019) did not have documentation of the required orientation training.

Plan of Correction: We will take the orientation Monday.
