Comments:
On 05/30/2019, starting at 9:45am and ending at 12:30 , an unannounced mandated inspection took place. On this day, three children were in care, totalling eight points. During the inspection, the children were observed playing with toys, washing hands, singing, free play, story time, and eating lunch. The provider was observed cooking and changing diapers. The inspector also reviewed 5 children's records and the provider's staff record. The inspector reviewed required posting, emergency plans and procedures, and the first aid kit as well.

Violations:
Standard #: 22VAC40-111-100-A
Description: Based on review of records, the provider failed to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: 1. Child (3) had a enrollment date of 07/27/2018 and their date for a physical examination was 05/28/2019.

Plan of Correction: The provider plans to ensure that the physical is obtained within the required amount of time.

Standard #: 22VAC40-111-320-B
Description: Based on observation, the provider failed to use child-resistant protective covers on all unused electrical outlets accessible to children under five years of age. Evidence: 1. An outlet located in the children's playroom was uncovered and located at floor level. Additionally, the children were observed in the room.

Plan of Correction: The provider replaced the outlet cover immediately.
