63.2 Licensure and Registration Procedures

Comments:
An unannounced monitoring inspection was conducted beginning at 12:45 a.m. and ending at 2:30 p.m. with the provider. There were four children in care ranging in age from 8 months to 4 years of age for a total of 10 points. Children were observed napping or resting quietly on their nap mats. An infant was observed waking, playing in a bouncer and drinking a bottle. Handwashing procedures were observed. Two children's records (including one new enrollee), one provider and one household member record were reviewed. Violations:
Standard #: 22VAC40-111-170-A
Description: Based on review of documentation, the provider did not obtain documentation of a Tuberculosis (TB) screening or test for an adult household member. Evidence: Household member #1 turned 18 on 3/8/2019 and there was no documentation of a TB test in the record.

Plan of Correction: A TB test/screening will be scheduled as soon as possible.

Standard #: 63.2-1721.1-B-2
Description: Based on review of documentation, the provider did not obtain documentation of fingerprint background checks for a household member. Evidence: Household member #1 turned 18 years of age on 3/8/2019 and did not have documentation of fingerprint background checks in the record.

Plan of Correction: The finger print background will be completed as soon as possible.
