Comments:
An unannounced monitoring inspection was completed on Monday, June 3, 2019 from approximately 10:45 AM to 1:30 PM. The provider was present with an assistant and 10 children, totaling twenty-six points. Children were observed engaging in activities to include outside time, transitioning to the restroom, lunch time and nap time. Children's equipment, learning materials, outdoor and indoor space, and restrooms were inspected. The provider, assistant, two household members and five children's records and a pet record were reviewed. Daily attendance records, fire drill, shelter in place, and all required postings were reviewed. Violations were cited and are documented on the violation notice. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Standard #: 22VAC40-111-890
Description: Based on observation, the provider did not ensure that food brought from home shall be clearly labeled in a way that identifies the owner. Evidence: Children in care had containers of Almond milk and Silk soy milk that was not labeled to identify the owner.

Plan of Correction: "Milk was labeled while inspector was here."

Standard #: 22VAC40-111-320-B
Description: Based on observation, the provider did not ensure that child-resistant protective covers shall be installed on all unused electrical outlets accessible to children under five years of age. Evidence: The electrical outlet on the lower part of the kitchen wall did not have a protective cover.

Plan of Correction: "Outlet was covered while inspector was here."
