Comments:
An unannounced monitoring inspection was conducted on June 14, 2019 from approximately 2:45 PM to 6:30 PM to determine the provider's compliance with licensing standards. In attendance were 11 children totaling 27 points being supervised by the provider and assistant. The children were observed interacting with the provider and the assistant, napping, washing hands, having snack, and playing with toys. Lunch today consisted of fish fillet, pineapple, corn, brown rice, and milk. Children's books, toys, and equipment were inspected and found to be age appropriate and in operable condition. The children's restroom was inspected with the required supplies present. The family day home had the required postings displayed. The first aid kit and emergency supplies were inspected with the required items found. There is no medication being administered at this time. Five children's records and two caregiver records were inspected. The following was also inspected: emergency drill logs, parent agreements, and other pertinent information. Violations:
Standard #: 22VAC40-111-880
Description: Based on observation, the licensee did not follow the most recent, age-appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA). Evidence: The licensing inspector observed the children were served rice cakes and water for the afternoon snack. At least two of the four nutritional components must be served for snack.

Plan of Correction: The licensee will ensure that milk is served during snack for the second required component.

Standard #: 22VAC40-111-100-A
Description: Based on a review of five children's records, two children's records did not contain documentation of a physical within thirty days after the first day of attendance. Evidence: The records for Child #2 (start date 9/1/18) and Child #3 (start date 5/14/18) did not contain documentation of a physical.

Plan of Correction: The licensee will request the documentation from the children's parents in order to update the records.

Standard #: 22VAC40-111-140-B-3
Description: Based on a review of two caregiver records, the licensee did not ensure that the provider shall have a current certification in cardiopulmonary resuscitation (CPR). Evidence: The record for Caregiver #1 contained documentation for CPR which listed an expiration date of 4/2019.

Plan of Correction: The licensee will ensure the certification is updated.

Standard #: 22VAC40-111-140-B-4
Description: Based on a review of two caregiver records, the licensee did not ensure that the provider shall have a current certification in first aid. Evidence: The record for Caregiver #1 contained documentation for first aid which listed an expiration date of 4/2019.

Plan of Correction: The licensee will ensure the certification is updated.

Standard #: 22VAC40-111-180-A
Description: Based on review of two caregiver records, two records did not contain documentation of a current Report of Tuberculosis (TB) Screening. Evidence: 1. The record for Caregiver #1 contained a TB screening which expired on 1/2/19. 2. The record for Caregiver #2 contained a TB screening which expired on 7/11/15. Each caregiver's record must contain documentation of a TB screening every two years, or more frequently as recommended by a physician.

Plan of Correction: The licensee will ensure the tuberculosis screenings are completed.

Standard #: 22VAC40-111-210-A
Description: Based on a review of two caregiver records, the licensee did not ensure that two caregivers obtained a minimum of 16 clock hours of annual training. Evidence: 1. The licensing inspector observed that the record for Caregiver #1 contained documentation for nine hours of annual training. 2. The record for Caregiver #2 (hire date 7/10/06) did not contain any documentation for annual training.

Plan of Correction: The licensee will ensure that caregivers complete 16 hours of annual training.

Standard #: 22VAC40-111-500-C
Description: Based on observation, the licensee did not ensure that stationary outdoor playground equipment shall not be installed over concrete, asphalt, or any other hard surface. Evidence: The licensing inspector observed the outdoor playground equipment with a slide and swings to be installed over dirt.

Plan of Correction: The licensee is having mulch installed under the playground equipment.

Standard #: 22VAC40-111-560-A
Description: Based on observation and interview, the licensee did not ensure that a caregiver shall provide direct care and supervision of each child at all times. Evidence: 1. The licensee identified Adult #1 as not being a caregiver. 2. Adult #1 was observed interacting with children in the back yard without a caregiver present. 3. Adult #1 was observed interacting with children in rooms in which there was no caregiver present.

Plan of Correction: The licensee will ensure that visitors will be observed by caregivers when children are present.

Standard #: 63.2-1720.1-B-2
Description: Based on review of two caregiver records, one record did not contain documentation of a national fingerprint-based criminal history record check. Evidence: The records for Caregiver #2 (hire date 7/10/06) did not contain documentation of a national fingerprint-based criminal history record check. For caregivers employed prior to 1/22/2018, a statement of eligibility based on a national fingerprint-based criminal history record check was required to be obtained prior to 9/30/2018.

Plan of Correction: The licensee will ensure an appointment is made for fingerprinting.
