54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced renewal inspection was conducted today between the hours of 9:40 am through 11:00. There were a total of 11 children (36 points) with 3 caregivers. A total of 4 children's files and 6 staff/household member's files were reviewed. The children were engaged in the following: toy play, baking a cake, outside play and hand washing. Standard #: 22VAC40-111-60-B Description: Based on a review of children records, all the information required is not in each child's record. Evidence: Child #1, date of attendance 4/8/19 does not have a copy of the physical exam. Child #2, date of attendance 12/17/18 does not have a copy of the physical exam and immunization record Plan of Correction: I have contacted both the parents. I will get the copies and put in their record.
Standard #: 22VAC40-111-180-A Description: Based on a review of household records, a current report of tuberculosis screening has not been updated every two years from the date of the first screening. Evidence: Household #1 does not have a current TB screening, the current TB screening is dated 1/5/17. Plan of Correction: I will get his TB screening and file the results in the record.
