Comments:
A subsidy health and safety inspection was conducted today with the provider who was present with an assistant and four day care children. There were no children enrolled with food allergies. Arrival time: 10:40 am Departure time: 12:45 pm

Violations:
Standard #: 22VAC40-665-230-B
Description: Based on a review of three caregiver's files, it was determined that caregiver A did not have the 10 hour of preservice training within 90 days of employment.

Plan of Correction: Caregiver A will get the training.

Standard #: 22VAC40-665-410-A-3
Description: Based on interview, it was determined that there was no annual lockdown drill.

Plan of Correction: I will do a lock down drill.
