Technical Assistance:
Licensing inspector discussed assistant and substitute provider's file requirements with Provider.

An unannounced monitoring inspection was conducted today from approximately 9:40am-11:30am. There were 6 children (14 points) in the direct care and supervision of 2 caregivers. A sample of 3 children?s records, 2 caregiver's records, and 2 household member's records were reviewed. The children were observed watching TV, playing with climbing equipment, napping, eating and playing outside. One household member has moved out since the last inspection. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Standard #: 22VAC40-111-60-A
Description: Based on interview, the provider failed to maintain an up-to-date record fro each child. Evidence: 1. Child B did not have a record on file. 2. Caregiver A stated she could not find the record for Child B.

Plan of Correction: I asked the mother if I gave it to her when the child was out of care for a few months. If she doesn't have it I will make another one.

Standard #: 22VAC40-111-60-B
Description: Based on record review, a child's file was missing required information. Evidence: 1 of 3 files reviewed were incomplete. Child A was missing the medical insurance policy number.

Plan of Correction: I'll ask the mother to complete it today. I will text her later.
