Comments:
A mandated Renewal inspection was conducted today from 10:10 am to 11:30 am. There were 2 children present (8 points) with 1 caregiver. Children were observed sleeping, having tummy time, diapering, hand washing, singing songs and learning baby sign language. Reviewed 3 children's files, 1 caregiver file, 1 resident file, emergency evacuation log, attendance, first aid supplies, crib compliance, food brought from home, and physical plant. Standard #: 22VAC40-111-60-B
Description: Based on record review, children's files did not contain required information,
Evidence:
1. Child#2 (start date 7/20/19) did not have documentation of parents' work phone numbers
2. Child#3 (start date 5/20/19) did not have documentation of child's medical insurance policy number

Plan of Correction: I will ask the parents for the information
