Training hours completed with classes scheduled in December.
Paperwork was collected from the provider regarding the LLC.
An unannounced renewal study was conducted today. There are 4 children present (12 points) with one adult at arrival. A second adult arrived within 20 minutes. 2 adult, 2 animal, and one children's records were reviewed. See Violation Notice form missing information. Children were observed resting, during diaper changing and playing with toys. Emergency escape drills, Shelter-in-place drills, first aid supplies, menu, emergency supplies and bathroom supplies are all in compliance.
Standard #: 22VAC40-111-240-A Description: Based on direct observation, a smoke detector is not operational.
A smoke detector in the home is "chirping" indicating a battery needs replaced.
Plan of Correction: Batteries will be replaced in the smoke detectors
Standard #: 22VAC40-111-460-C Description: Based on a review of documentation, rabies vaccinations are not current for two animals.
2 of 2 animals residing in the home have expired rabies vaccinations. Documentation available expired 11/06/2019
