Technical Assistance:
Standard 580.C was reviewed:
Daily age-appropriate activities shall include opportunities for vigorous outdoor play daily, depending upon the weather, the ages and the health of the children.
Standard 840.A was reviewed - Documentation of children's injuries.
USDA guidelines reviewed to include the child care food program meal pattern guidelines.
*Discussed removing the string from the backyard before resuming use of this area for outdoor play.

An unannounced monitoring inspection was conducted on 11/21/2019 from 10:05 am - 11:45 am. There were nine children in care with three caregivers. The ages of the children counted for a total of 20 points. One resident child was in care at the beginning of the inspection and the age of this child counted for an additional three points.
Children were observed during circle time which included story telling, review of letters and numbers and a flannel board activity.
Children were also provided with table activities using building blocks.
Records were reviewed for three caregivers and one household member. Seven children's records were reviewed.
The outdoor play area was reviewed however, the provider stated that the backyard was not in use at this time.
The violations are listed on the violation notice and were reviewed with the provider at the conclusion of the inspection.

Standard #: 22VAC40-111-120-B
Description: Based on record review, the provider failed to ensure that assistants' records contain documentation that two or more references as to character and reputation as well as competency were checked before employment.

1. There were no reference checks on file for caregiver 2 who was working as an assistant during the inspection.
a. Caregiver 2 has a hire date of 06/20/2019.

Plan of Correction: Reference checks were completed for this individual however, they have been misplaced. The reference checks will be re-created and placed in the file of caregiver 2.

Standard #: 22VAC40-111-60-A
Description: Based on record review, the center failed to ensure that children's records contain all of the required elements.

1 The enrollment record for child 1 lacked the name, address and phone number for a second emergency contact.
2. The enrollment record for child 2 lacked an address for the second emergency contact.

Plan of Correction: The parent of child 1 and child 2 will be asked to provide complete emergency contact information.

Standard #: 63.2-1720.1-B-3
Description: Based on record review and interview, the provider failed to obtain a copy of the results of a search of the central registry maintained pursuant to ? 63.2-1515 and any child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided in the preceding five years for any founded complaint of child abuse or neglect against him.

1. There was no out-of-state central registry check on file for caregiver 1 who indicated on her sworn statement of affirmation that she had resided in Florida within the page five years.
a. Caregiver 1 has a hire date of 04/29/2019.
2. There was no out-of-state central registry check on file for caregiver 2 who indicated on her sworn statement of affirmation that she had resided in North Carolina within the page five years.
a. Caregiver 2 has a hire date of 06/20/2019.
3. The provider confirmed that an out-of-state central registry check had not been obtained for both caregivers.

Plan of Correction: An out-of-state CPS check will be requested for both caregivers by Friday, November 29th.
