Violations:
Standard #: 22VAC40-111-430-A
Description: Based on observation and provider statement, children under two years of age have access to the stairs as no gate was in use upon arrival.
Evidence:
The provider did not have a gate at the bottom of the steps to prevent access to the stairs for the children in care.

Plan of Correction: Not available online. Contact Inspector for more information.

Standard #: 22VAC40-111-430-B
Description: Based on observation, an accordion expansion gate was placed at the stair opening.
Evidence:
When the provider put a gate in use it was an expansion gate that was not hardware mounted to the wall to prevent access to the stairs or that would give away under a child's weight.
