Comments:
An unannounced renewal inspection was conducted today, 11/18/19. The provider does not have any children registered for care.
A review of standards was completed, there are several standards that cannot be determined due to no children in care.
We discussed that the Sworn Disclosure statements.

Inspection began: 1:15 pm and ended 2:45 pm.

Standard #: 22VAC40-111-330-A
Description: Based on interviews and observation the provider failed to have a landline telephone.
Evidence:
1. The provider stated that she does not have a landline phone at this time.

Plan of Correction: The provider has a cellphone. The provider will check into getting a landline phone.
