Technical Assistance:
Standard 320.E was discussed - potable electric heaters shall be in areas inaccessible to children and located at least three feet from combustible materials.

Comments:
An unannounced renewal inspection was conducted on 01/06/2020 from 10:43 am - 12:00 pm. There were four children in care with the provider. The ages of the children counted for a total of 11 points. Children were observed playing with toys in the playroom with the provider. Records were reviewed for four children, the provider and one household member.
The entire home and backyard play area was reviewed.

Violations were observed in two parts of the FDH standards and are listed on the violation notice.

Violations:
Standard #: 22VAC40-111-800-B
Description: Based on record review, the provider failed to review the emergency plan at least annually and update the plan as needed.

Evidence:
The emergency preparedness plan for the family day home was last reviewed by the provider on 09/10/2018.

Plan of Correction: The emergency plan will be reviewed and updated, if needed, within the week.

Standard #: 22VAC40-111-100-A
Description: Based on record review, the provider failed to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Evidence:
1. There was no documented physical on file for child 1.
a. Child 1 was in care during the inspection and has an enrollment date of 11/20/2019.

Plan of Correction: The parent will be asked at pick-up to obtain a current physical within the next two weeks.
