Technical Assistance:
Licensing inspector discussed hours of operation with the provider.

Comments:
An unannounced renewal inspection was conducted today from approximately 10:50am-12:10pm. There were 2 children (5 points) directly supervised by 2 caregivers. A sample of 2 children?s records, 2 caregiver's records, 2 household member's records and two pets records were reviewed. The children were observed playing with toys, watching television ad engaged with caregivers. Zero new caregivers have been hired since the last inspection. Description: Based on record review, the provider did not review each file with the parent.

Evidenced:
The file of Child A and Child B was last reviewed 10/19/2018.

Plan of Correction: Parent will review the file today.

Description: Based on observation, emergency numbers were not posted.

Evidence:
911 was not posted in the facility.

The vaccination for the Cat expired 01/08/2020.
