- Technical Assistance:
Discussed transitioning the three year old children from the crib to a cot. Since the cribs are in the same area as the play space, the children to his will allow more space in the room for play. Suggested purchasing another table for the children, spreading them out to avoid biting which was observed today.
An unannounced monitoring inspection was conducted today between the hours of 9:05 am through 10:45 am. There were a total of 5 children, including one of Provider's(13 points) observed with 2 caregivers. A total of 4 children's files and 3 staff/household member's files were reviewed. The children were engaged in the following: toy play, coloring diapering, hand washing and word game. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-111-100-A Description: Based on a review of children records, the provider has not obtained documentation of a physical examination within 30 days after first day of attendance.
Evidence: Child #1, date of first attendance, 10/7/2019 does not have documentation of a physical examination.

Standard #: 22VAC40-111-180-A Description: Based on a review of caregiver and household records, the provider has not obtained every two years from the date of the last screening, an updated Tuberculosis screening.
Evidence: The following have not updated their TB screening, caregiver #1 TB dated 1/29/18, caregiver #2 TB dated 11/6/17 and household # 3 TB dated 1/31/18.
Plan of Correction: We will get our TB screenings.
Standard #: 22VAC40-111-280 Description: Based on observation, potentially poisonous substances were stored in an area accessible to children.
Evidence: Cleaners and hand sanitizer were found accessible to children on top of the dryer in the bathroom,
Plan of Correction: I will put in cabinet away from children.
Standard #: 22VAC40-111-690-D Description: Based on observation, the diapering surface is not nonabsorbent.
Evidence: The pad used for diapering has 2 small cracks on both sides, making it nonabsorbent.
Plan of Correction: I will buy a new pad.
