Standard #: 22VAC40-111-60-A Description: Based on a review of children's records, the Family Day Home failed to maintain an up to date records for each child enrolled.
Evidence:
1. Child #2 and # 4's records do not contain documentation of names of person other than the custodial parents who are authorized to pick up the child.
2. Child #3's record did not contain documentation of an annual information update. The last update was 01/02/2019.
3. Child #2's record did not contain written authorization for emergency medical care should an emergency occur.Plan of Correction: I will get the information from parents.
Standard #: 22VAC40-111-180-A Description: Based on a review of caregiver records, the Family Day Home failed to obtain for each caregiver a current Report of Tuberculosis Screening form in accordance with the requirements in 22 VAC 40-111-170 every tow years from the date of the first screening or more frequently as recommended by a physician or the local health department.
Evidence:
Caregiver #3 Tuberculosis Screening has expired.Plan of Correction: I will have caregiver obtain new screening.
