The licensing inspector conducted an unannounced renewal inspection October 20, 2014 from approximately 9:30am to 2:00pm. The following ratios were observed with a total of 132 children present during the inspection: Young Infants - 7 children with 2 staff Older Infants -7 children with 2 staff Toddlers (Red Room) - 15 children with 3 staff Twos (Orange Room) - 16 children with 3 staff Twos (Yellow Room) - 19 children with 2 staff Threes (Blue Room) - 14 children with 2 staff Threes (Green Room) - 19 children with 2 staff Fours & Fives (Purple Room) - 17 children with 3 staff Fours & Fives (Gold Room) - 18 children with 2 staff Children were not present in the Satellite Infant Room or Get Well The children were observed engaged in a variety of activities during the inspection. The Infants were observed eating, sleeping, and playing on the floor with toys. The Toddlers were observed eating lunch. The Twos were observed playing on outside, having books read to them, building with blocks, playing in Station Square and having their diapers changed. The Threes were observed painting pumpkin cutouts with sponges, playing outside, and creating their own individual ?monsters?. The Fours and Fives were observed playing outside, creating scarecrows out of clothespins, creating scarecrow masks, playing outside and participating in various centers located in the classrooms. Lunch consisted of cheesy black bean and rice casserole, corn, oranges, and milk. All areas of the center including classrooms, hallways, bathrooms, and playgrounds were inspected. The following were reviewed during the inspection: injury reports, staff training information, emergency supplies, emergency and evacuation drills, daily attendance, daily sheets, and menus. Medication is administered and medication and authorization forms were reviewed. During the inspection, six children records and nine employee records were reviewed. The risk profile tool will be used for the licensing recommendation once all of the necessary paperwork is received by the licensing department. The renewal application was received prior to the expiration of the current license. Until the new license is received, the provider is legally operating. Standard #: 22VAC40-185-330-B Description: Based on observations and measurements taken by the licensing inspector, the center did not maintain the required depth of resilient surfacing in the fall zone areas around playground equipment. Evidence: During the inspection, the licensing inspector measured the following: 1. The depth of mulch at the end of the slide on a piece of climbing equipment located on the Toddler playground measured approximately four inches in depth. The slide/climbing equipment measured approximately three feet in height and required six inches of mulch in the fall zone areas. 2. Measurements of the mulch were taken by the four swings located on the Preschool playground. The depth of mulch located approximately six inches in front of the first swing was approximately one inch in depth. The depth of the mulch measured directly behind the first swing measured approximately two inches in depth. A required depth of nine inches was required in the fall zones areas for the swing set. During the inspection, a staff member raked additional mulch into the fall zones bringing the resilient surfacing in the fall zone areas in compliance. Plan of Correction: Per director - This was corrected during the inspection. We will retrain staff to ensure the mulch is being redistributed to the fall zone areas.
Standard #: 22VAC40-185-510-G Description: Based review of medication and medication authorization, the center did not ensure all prescription medication present at the center contained a pharmacy label. Evidence: During the inspection, the licensing inspector observed an Epi-Pen that contained a hand-written label containing a child's name. Prescription medication is required to contain a pharmacy label including the child's name the medication is being prescribed. Plan of Correction: Per director - The family brought in the pharmacy label for the medication the next day.
Standard #: 22VAC40-185-520-C Description: Based on observation, the center did not ensure all diaper ointments were labeled with a child's name. Evidence: During the inspection of the Infant room, one diaper ointment was not labeled with a child's name. The diaper ointment was labeled by a staff member during the inspection. Plan of Correction: Per director - Diaper cream was labeled during the inspection. I will retrain staff to label diaper creams when they come in.
