Comments:
An unannounced monitoring inspection was conducted from 8:30 am-12:00. At the time of entrance, 138children were in care with 19 staff members present. Children were observed during snack time, engaged in group activities and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. A complaint investigation was also conducted this date, a violation for physical plant was cited with the complaint investigation. The violations were reviewed with the center director. Standard #: 22VAC40-185-130-A
Description: Based on record review, licensee did not obtain a child's immunization record before the child attended the center. Evidence: Child #1 was enrolled 10/3/14, the record did not have documentation of the child's immunization record before the child's first day of attendance.

Plan of Correction: Parents returned immunizations/ physical to center with updated information.

Standard #: 22VAC40-185-140-A
Description: Based on record review, licensee did not obtain a child's physical examination within one month after attendance. Evidence: Child #1 was enrolled 10/3/14, the record did not have documentation of the child's physical examination as of this date, 11/13/14.

Plan of Correction: Parents returned immunizations/ physical to center with updated information.

Standard #: 22VAC40-185-350-E-2
Description: Based on observation and staff interviews, ratio for children 16 months; one staff for every five children was not maintained. Evidence: The Licensing Inspector entered room # 3, the toddler room around 9:45am. There were 2 staff with 11 children, ages 16 months and above. Behind me entered a father dropping off his toddler, the father immediately asked what was the ratio for the classroom. At the same time the father entered, a staff was on the phone asking for another teacher, who arrived within a minute. After asking a staff member how long they were operating with 11 children before my arrival, she responded about 5-10 minutes.

Plan of Correction: Adjusted schedules of teachers, as well as training the teachers that when they get close to ratio, call so a manager can make sure there is adequate staff to support ratio.

Standard #: 22VAC40-185-510-N
Description: Based on a review of medication, licensee did not notify a parent within 14 days of the medication authorization expires. Evidence: Child # 2 medication authorization to administer an epi pen expired 9/12/2014.

Plan of Correction: Sent home immediately to parents with new paper work to be filled out by doctor.

Standard #: 22VAC40-185-520-C
Description: Based on observation and staff interviews, licensee did not obtain written parent authorization for a child's diaper ointment. Evidence: Child #3, who was in attendance this date had diaper ointment without written parent authorization.

Plan of Correction: Had parent fill out diaper cream form.
