Technical Assistance:
Discussed changes to the MAT certificate. Discussed purging the communication forms (420.E) used for infants through twos, the TA dated 11/2012 does not require that these forms are kept for 2 years after termination.

Comments:
An unannounced monitoring inspection was conducted from 8:45 am-11:30. At the time of entrance, 67 children were in care with 23 staff members present. Children were engaged in group activities; sensory table, matching, play dough, puzzles, music and gross motor play time. A selection of staff and children records, medications and other documents were reviewed. The violations were reviewed with the center director. Standard #: 22VAC40-191-60-C-2
Description: Based on review of staff records, licensee has not obtained a central registry finding within 30 days of staff's employment. Evidence: Staff #1, hired 11/2013 does not have documentation of central registry finding on file and is still actively working at the child care center.

Plan of Correction: Human Resource sent original in 11/2013 and it was resubmitted again on 5/6/14. The center director will have staff complete another form and resubmit. The center director will follow up with Human Resource within 30 days for the response.
