An unannounced monitoring inspection was conducted today. There were 35 children in care, with a total of 4 staff members assigned to their direct supervision. This inspection took place during a public school closure day. The children were observed playing active group games in the gym and preparing for their afternoon Superbowl feast. Standard #: 22VAC40-185-60-A Description: Based on a review of records, it was determined that one child's record was not complete. Evidence: Child record #3 does not include a shot record. Plan of Correction: I will follow-up with the school nurse and get a copy of the child's health records.
Standard #: 22VAC40-185-270-A Description: Based on observations, it was determined that one area used by the children has not been maintained in a clean condition. Evidence: Inside the children's bathroom in Room 2, the scooper used to clean out the bunny's cage has been placed on the floor next to the toilet. Plan of Correction: I will move the scooper to the closet.
Standard #: 22VAC40-185-510-A Description: Based on a review of children's medications, it was determined that one child's medication authorization form is not complete. Evidence: The authorization form for one child's long-term medication does not include the parent's signature. Plan of Correction: I will ask the parent to sign the form.
Standard #: 22VAC40-185-510-C Description: Based on a review of children's medications, it was determined that one child's medication authorization orders have not been followed. Evidence: The authorization form for one child's prescription medication indicates that two doses of the medication shall be kept on site. However, there is only one dose of medication on site and readily available for use by SACC personnel. Plan of Correction: The parent has already been asked to bring a second medication. I will remind the parent to bring it.
