The licensing inspector conducted an unannounced monitoring inspection February 25, 2015 from approximately 10:00am to 12:30pm. The following ratios were observed with a total of 54 children present during the inspection: Infants -6 children with 2 staff Twaddlers (Older Infants) - 8 children with 2 staff Toddlers - 5 children with 1 staff Twos - 7 children with 1 staff Multi-Age (Twos & Threes) -10 children with 2 staff Preschool & Pre-K - 18 children with 2 staff School Age children were not present during the inspection The children were observed engaged in a variety of activities during the inspection. The Infants were observed being rocked, sleeping, having books read to them, staff members singing to them, and playing with toys. The Toddlers were observed having their diapers changes and playing with balls. The Twos classroom was observed creating a Pink Collage using a variety of pink items, playing with cars on a road rug, reviewing the sounds different animals make, playing with baby dolls and dancing. The Multi-Age classroom was observed singing, listening to music, playing with cars and building with blocks. The Preschool/Pre-K classroom was observed participating in learning centers including cutting pictures from magazines, painting with watercolors, creating bracelets using beads and pipe cleaners, lacing shapes, and playing with legos. Staff was observed engaged in conversation with the children and assisting children with individual needs. Lunch consisted of fish sticks, mixed vegetables, tropical fruit and milk. All areas of the center including classrooms, hallways, kitchen and bathrooms were inspected today. The center was equipped with toys and supplies and was available to the children. Playgrounds were not inspected due to weather conditions. The following were reviewed during the inspection: injury reports, emergency supplies, emergency and evacuation drills, daily attendance, daily sheets, and menus. Medication is administered and medication and authorization forms were reviewed. During the inspection, five children records were reviewed. Staff records are stored at an off-site location and will be reviewed at a later date. Standard #: 22VAC40-185-140-A Description: Based on review of five children's records, one record did not contain documentation of a physical exam. Evidence: During the inspection, the licensing inspector observed the record for Child #1 (enrollment date 08/11/14) did not contain documentation of a physical examination by or under the direction of a physician. Plan of Correction: Per director - We will reach out to the family and have them obtain a copy of the physical exam to be added to the record.
