The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing department on 02/04/2015. The inspection was initiated on 02/12/2015 and completed on 02/18/2015. On 02/12/2015, the inspector was on site from 7:50am to 10:30am. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. The preponderance of the evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. In addition to the original complaint, there was also a violation found during the inspection regarding staff qualifications. See the violation notice for more details. Description: Based on review of one employee's record, the program director's record did not contain documentation of job qualifications for the job position. Evidence: During the inspection, the licensing inspector observed the record for Staff #1 (hire date 09/09/14) did not contain documentation of a bachelor's degree in a child -related field.

Description: Based on observation, the center did not ensure the required staff to Infant child ratio was being followed. Evidence: During the 02/12/15 inspection, the licensing inspector arrived at the center at approximately 7:45am. Once entering the Infant room, the licensing inspector observed 10 Infants present with two staff members from 7:45am until the 8:15am. A third staff member arrived at approximately 8:15am and transitioned three children into the Toddler room. Centers are required to maintain a one staff member for every four Infants present at all times. Based on observations an additional staff member was required.

Plan of Correction: Per director - A teacher has returned from disability leave and opens in the morning to ensure ratio is intact. I have made an offer to a lead teacher on 03/03/15 and currently interviewing more applicants to hire additional teachers.
