Comments:
The licensing inspector conducted an unannounced renewal inspection March 9, 2015 from approximately 10:45am to 1:30pm. The following ratios were observed with a total of 48 children present during the inspection: Infants - 8 children with 3 staff Ones - 3 children with 1 staff Ones - 7 children with 2 staff Twos - 10 children with 2 staff Threes - 4 children with 1 staff Threes - 6 children with 1 staff Fours - 10 children with 1 staff School age children were not present during the inspection. The children were engaged in a variety of activities when the licensing inspector arrived. During the inspection the children were observed being held and backs being rubbed for naptime. Children were also observed coloring, working puzzles, playing with blocks, singing songs, marching and dancing. Diaper changes, potty breaks, and lunchtime were observed. Lunch consisted of chicken nuggets, corn, applesauce, potato wedges, and milk. All areas of the center including classrooms, hallways, bathrooms, kitchen, and playgrounds were inspected. The following information was reviewed: emergency supplies, fire and shelter-in-place drills, parent agreements, liability insurance coverage, and staff training information. The manager stated medication is not present at this time. Transportation is provided and vans and transportation documentation were reviewed. During the inspection, five children records and four employee?s records were reviewed. The risk profile tool will be used for the licensing recommendation once all of the necessary paperwork is received by the licensing department. The renewal application was received prior to the expiration of the current license. Until the new license is received, the provider is legally operating. Standard #: 22VAC40-185-140-A
Description: Based on review of five children's records, one record did not contain documentation of a physical exam. Evidence: During the inspection, the licensing inspector observed the record for Child #2 (enrollment date 01/05/2015) did not contain a physical examination by or under the direction of a physician.

Plan of Correction: We will have the family fax a copy of the child's physical to the center.

Standard #: 22VAC40-185-60-A
Description: Based on review of five children's records, one record did not contain the required information. Evidence: During the inspection, the licensing inspector observed the record for Child #2 (enrollment date 01/05/2015) did not contain the address for two emergency contacts.

Plan of Correction: We will ask the family to provide the physical address for the two emergency contacts listed in the child's file.

Standard #: 22VAC40-185-70-A
Description: Based on review of four staff records, one record did not contain the required information. Evidence: During the inspection, the licensing inspector observed the record for Staff #2 ( hire date 09/05/2014) record did not contain two character references checks. Two reference checks are required to be obtained prior to a staff member beginning employment.

Plan of Correction: References were obtained during the inspection.

Standard #: 22VAC40-185-440-J
Description: Based on observation, the center did not ensure there were at least twelve inches of space between the sides and ends of occupied cribs. Evidence: During the inspection, the licensing inspector observed six children sleeping in cribs. The ends of the cribs were pushed against each other.

Plan of Correction: During the inspection the manager moved the cribs so the cribs had the required spacing.
